Review and Complete Your Service Agreement

Please review and complete the Client Service Agreement before your appointment with Everwell Veterinary. This document outlines expectations, policies, and the scope of care, helping ensure a transparent and professional experience during your in-home visit.

This Concierge Client Service Agreement (“Agreement”) is made between Everwell Veterinary (“Provider”) and the undersigned pet owner (“Client”). By electronically signing below and enrolling in Provider’s subscription service, Client agrees to the terms and conditions set forth here.

This Agreement is intended to protect the safety, well-being, and comfort of the patient, Client, and Provider while enabling high-quality, compassionate in-home veterinary care.

Appointment Logistics and Presence Requirement

Philosophy of Care, Comfort, and Procedural Consent

Low-Stress Handling: Provider practices high-touch, low-stress, in-home medicine. Provider will use gentle handling and low-stress techniques.

Safety Protocols: If the patient appears uncomfortable, distressed, or if safe handling cannot be achieved, Client agrees to discuss and, if indicated, permit additional measures (including, but not limited to, anxiolytics, sedation, or alternative handling techniques) to ensure the safety and welfare of the patient, Client, and Provider.

*Discounts will be given for multi-pet households on all tiers.

Emergency Care: Provider will explain options and obtain consent when feasible; in an emergency, Provider may act without prior consent to protect life or prevent serious harm.

Cancellations and Late Arrivals

Policy Abuse: Repeated late cancellations or no-shows may result in suspension or termination of services.

Late Policy: If the Client is more than 15 minutes late without prior notice, the Provider may treat the visit as a cancellation and charge the applicable fee.

Communication Policy

Confidentiality: Provider will respect Client confidentiality and comply with applicable privacy practices.

Referrals and Continuity of Care

Referrals: Provider may recommend referrals to specialists or emergency clinics when clinically indicated.

Risks, Limitations, and Release of Liability

Indemnification

Termination

Written Notice: Either party may terminate this Agreement with written notice. Termination does not relieve Client of payment obligations for services rendered or subscription fees due prior to termination.

Immediate Termination: Provider may terminate services immediately for safety concerns or repeated breaches of this Agreement.

Governing Law and Dispute Resolution

Governing Law: This Agreement is governed by the laws of the State of Mississippi.

Arbitration: Any dispute arising under this Agreement shall be resolved by binding arbitration in Madison County, Mississippi, under the rules of the American Arbitration Association (or state arbitration forum), unless otherwise agreed in writing.

Amendments

Provider may amend this Agreement from time to time. Provider will notify Client of material changes, and continuation of services after notice constitutes acceptance of the amended terms.

Electronic Signature and Consent

By electronically signing below, Client certifies that they are the owner or authorized agent for the patient, have read and understood this Agreement, agree to its terms, and consent to in-home veterinary care under these conditions. Client further authorizes automatic monthly billing for the subscription plan.
